Describe your organization: Did you know that more than 60,000 people in Dane County alone are in need of dental care? More Smiles Wisconsin helps bridge that gap by providing quality dental care to children, adults, and seniors in South Central Wisconsin. We are a non-profit, community dental clinic that provides a wide range of dental services to residents on BadgerCare and those without insurance. Founded in 2009, More Smiles hosts a downtown clinic and emergency center and is in the process of building a Fitchburg-West Side location.
In addition to our direct care services, More Smiles also has a number of specialty programs. We are growing an educational arm to provide community and patient oral health education. This service seeks to improve oral health literacy so we can tackle the root cause of many dental health problems, reducing overall demand. We also lead care coordination for the Dane County public health's Dental Care Diversion Program. This program seeks to redirect non-traumatic dental issues from area ERs and urgent care centers, helping to save up to $2.5 million in unnecessary care and reducing opioid usage.

Describe your organization: We lead the fight against the deadly consequences of diabetes and fight for those affected by diabetes.
The American Diabetes Association-Wisconsin supports those living with type 1, type 2, and gestational diabetes. We offer educational opportunities for the medical professional as well as for those living with diabetes, including an annual Diabetes Educational Conference, a residential camp for children living with type 1 diabetes, a newly diagnosed family retreat for children diagnosed with type 1, as well as many community initiatives for type 1 and type 2 individuals within our communities. We also have a fundraising campaign, the Tour de Cure, which engages fundraisers with a walk, run or biking option to participate in a day of fun and celebration honoring loved ones living with diabetes.

Describe your organization: Kennedy Heights Community Center (KHCC) is a non-profit community center on Madison's north side. The KHCC is located within the Kennedy Heights housing complex and serves people in the neighborhood and the greater north side. Describe your organization: We are a 34-year-old non-profit that has programs of investment and opportunity creation for working poor persons of Latin America. Our mission is to create opportunities for access to microfinance, markets and services to improve the lives of the working poor and their communities in Latin America. Our largest program is the Capital for Communities Fund that is comprised of approx. $12-13M of social investors' and WCCN funds that are aggregated and then lent to community partner organizations based in 6-9 countries of Latin America. These partner organizations receive loans from WCCN, not grants, to provide micro-loans and fair trade pre-harvest financing to micro-entrepreneurs and smallholder farmers in their low-income communities. These micro- and pre-harvest loans range in size from $11-2500 on average. Poor people are our source of repayment, and this means that our model of financing them is a risk-based model but one that has consistently returned a repayment stream for more than 25 years. We have a 100% repayment rate to our investors, and we have about 10% of our revenues from donations. We selectively make small grants, but they are minimal compared to our loan portfolio. It's rewarding and intense work. The organization shares the risks of our clients, whatever that means: ---climate change, government coups or arbitrary interventions, commodity price drops, crime, conflict, everything. So, their loss can mean our loss. If you are up for that kind of experience, then give us a try. You are helping people who make between $2-10 a day live in dignity and change the poverty dynamic in their families and communities.

Describe your organization: Started in 2007, REACH-A-Child began with a mission of providing books and backpacks to First Responders in Wisconsin. The rationale was to ensure that these professionals have a tool to use when encountering children in crisis. Sine our inception, hundreds if not thousands of children have been comforted by Police, Fire, EMS, Sheriff and State Patrol personnel through the power of a book. REACH-A-Child partners with individuals and businesses in communities across Wisconsin to fund these resources - which then go back to the community as books and backpacks for the First Responders.
More recently, we have also provided Wisconsin's First Responders with large quantities of children's books for use in non-crisis, positive interactions with younger citizens. This action helps children build their personal library and strengthens their trust in the First Responder community.

Describe your organization: For over 90 years, Easter Seals Wisconsin has provided a variety of programs for people with disabilities including vocational rehabilitation, respite services, camping and recreation, information and referral, and public education. Services are designed to help people with disabilities live independently, become self-sufficient and enjoy life more fully.
One of the oldest, charitable organizations in the state, Easter Seals Wisconsin currently serves close to 10,000 children and adults with disabilities and their families each year throughout Wisconsin. Our work is first and foremost about the people we serve, their families and their communities. Our primary programs are:
Camp Wawbeek is the longest continuously-operating Easter Seals camp in the nation. While fun is the top priority, Camp Wawbeek also provides a fully accessible camp environment for children and adults with disabilities as well as a year-round network of social support.
Easter Seals Respite programs offer a safe and nurturing place where campers are provided with the care and support they need while their parents and caregivers can take a break from the day-to-day caregiving role.
Veteran's Family Camp is offered twice a year and free to veterans and their families. Offered in May and September, it is a relaxing weekend meant for families to reconnect and network with other veterans, while learning about resources available to them.
Since 1991, the FARM Program specializes in helping farmers return to farming after a disabling illness or injury, or continue farming with a chronic condition. It is a national model for similar programs across the country.
Self-Employment Services helps clients develop and evaluate their business ideas according to both business feasibility and issues presented by their disability.
Describe your ideal board member : Board candidates are generally leaders in their communities, individuals with vision and good business judgment who are invested in the agency's mission and willing to advocate for Easter Seals Wisconsin and individuals with disabilities. We are specifically looking for at least one person with a marketing or public relations background, but are open to others based on qualifications, interest and experience.
Board members are asked to give personally; to actively participate as a member of at least one board committee: to serve as a volunteer fundraiser in our Annual Campaign; to attend major agency events, e.g. biennial Recognition Dinner (next is fall 2018), to Annual Campaign Kickoff event (April); to participate or volunteer in Golf Outing for Kids, our signature event held in Wisconsin Dells (June); and to help to further the organization through the benefit of their particular interest and expertise and by opening doors in their communities and influencing others to support Easter Seals Wisconsin and its various programs.
Board members should have the flexibility and commitment to attend meetings four times per year, usually held on Fridays. Our board meets three times in Madison and once in Wisconsin Dells (July). Committee meetings are held in the morning, followed by lunch and ending with the board meeting in the afternoon. Describe your organization: Women In Transition serves adult women who have a serious and persistent mental illness. Women In Transition is composed of three programs, a Community Based Residential Facility, a supported apartment program which is situated next to the CBRF, and a satellite program where consumers live on their own but with the support of the WIT social work staff.
The staff strives to provide flexible, individually tailored services. Staff is available to listen and respond with information on available services, provide support, counseling and problem-solving. We believe in the concept of empowerment. Individual goals and personal desires are paramount in clinical planning and most decision making. Staff actively encourages a cooperative living arrangement and highly value individual desires and encourage women to express their opinions and wishes. However, at times, the need to provide an organized and supportive environment for the group may override an individual request or desire.
WIT has established participant expectations and responsibilities that are essential to the program. They include integration of independent living skills as well as socialization skills. Response plans allow for person-centered planning based on a partnership between provider and consumer.

Describe your organization: UNIDOS started in 1996 after research conducted with migrant farm worker communities in WI indicated that, although domestic violence was a serious problem within the Latino migrant and immigrant community, local DV services did not have the resources to work with this group of abused women. UNIDOS has become a leader around the state as a resource for training and technical assistance to a variety of agencies working with abused members of the Latinx community. We assist clients in a variety of tasks such as divorce, child custody, court accompaniment, etc. LA RED, our help-line, is currently the only Spanish-Language help-line in the county for victims of abuse. We also offer support groups for survivors to learn healthy coping strategies and decrease isolation through peer support.

This year, Make-A-Wish® Wisconsin granted more wishes than ever before - 400 wishes. But there is still more work to be done. Describe your ideal board member: The primary purpose of the Madison Regional Advisory Council (MRAC) is to strengthen local associations and build deeper connections in South Central Wisconsin. The MRAC acts as an auxiliary group of leaders that actively promotes awareness, initiates and supports fundraising efforts, and provides direction regarding growth opportunities within the region.
The MRAC is comprised of well-connected community members wishing to make a lasting impact in the lives of children with life-threatening medical conditions. Breast Cancer Recovery's mission is to provide environments for women breast cancer survivors to heal emotionally. All programs are designed and conducted by survivors for survivors.
Programs
Breast Cancer Recovery offers wellness retreats- Infinite Boundaries- for women in all stages of breast cancer - newly diagnosed, in treatment and many years finished with treatment. Women ages 21 - 80 have attended our retreats. Breast Cancer Recovery also offers an annual educational program, Sharing the Knowledge.
History
Founded in 1997 by Ann Haney, Breast Cancer Recovery is a member of the Wisconsin Cancer Council, and the Wisconsin Breast Cancer Coalition. Breast Cancer Recovery networks with hospital staff, breast cancer support groups, nursing and oncology nursing associations, and hospital auxiliaries throughout the state of Wisconsin, and the upper Midwest.

We are the Wisconsin Chapter of Amigos de las Américas, an international nonprofit organization, with headquarters in Houston, TX. Founded in 1965, AMIGOS has established a strong network of partners across Latin America making it possible to provide authentic community engagement and cultural immersion for our program participants. In our 50 year history, we have had 26,000 alumni complete our programs with a greater social awareness, stronger leadership skills, and increased passion to be agents of social change. Our AMIGOS- WI chapter connects directly with Madison area High School students. We support both the families and students through a training and preparation process prior to the student volunteers' summer departures to Latin America.

Describe your organization: The Home Health United – Visiting Nurse Service Foundation supporting SSM Health at Home provides assistance across southern Wisconsin to those facing health related issues.The programs our Foundation supports include: Hospice and Bereavement services, including Camp GLOW, the SSM Health Hospice House Resident Fund, Meals On Wheels, Granting Wishes, Telehealth services and any other area of great need that is recognized by our staff or the community.
Our Hospice Program supports our Volunteer and Grief Support programs (both of which are required by Medicare/Medicaid but are not reimbursed), upgrades and maintenance of the SSM
Health Hospice House residential hospice facility, and capital needs of the Hospice program.
While most people would prefer to stay at home during their final days, sometimes that is not possible. The SSM Health Hospice House Resident Fund covers room and board costs at the SSM
Health Hospice House for those who are unable to afford them. While Medicare, Medicaid and most private insurance covers care associated with hospice services, they do not cover room and
board costs for residential facilities. The Foundation ensures that anyone, regardless of their financial situation, has access to the peaceful, home-like setting provided by the SSM Health
Hospice House.
SSM Health at Home's Meals On Wheels Program ensures that over 300 people in the Madison, Monona, Middleton, and Sun Prairie communities receive a hot, nutritious meals, 5 days a
week along with an ever important well check. Without the support of the Foundation this program would not exist as the majority of the cost is subsidized by fundraising and donations.
Children and teens often experience and express grief in different ways than adults. Camp GLOW a grief support day camp for kids who have been affected by a loss. The camp is
designed to give children a safe place to share their feelings and concerns with a wide range of activities and enough counselors on-site to give individual attention if needed. The Camp is
offered at no charge to the participants and their families.
SSM Health at Home offers a wide range of services – all with one goal – to keep our patients as safe and independent as possible at home. And, the one goal of our Foundation is to ensure no person is turned away from the care they need due to their financial situation.

Literacy Network is a non-profit organization in Madison, WI founded in 1974. We work with teachers, tutors, volunteers and donors to improve adult literacy in Dane County. Our programs help adults find career opportunities, read with children and help with homework, improve computer skills, understand finances, and speak with doctors about health concerns. The Literacy Network programs include classes and tutoring programs throughout Dane County which engage more than 500 volunteer tutors and serve more than 1,000 adult learners each year. Describe your organization: The Employment and Training Association (EATA) is an established private, non-profit, community based organization providing employment and training services in South Central Wisconsin since 1966. EATA operates a variety of federal, state, county and locally funded training programs in the region including: the Children First program serving non-custodial parents; the Windows to Work program, a pre- and post- release program serving individuals re-entering the community from Wisconsin Department of Corrections; and the LEAP program serving individuals pre- and post- release from the Dane County Jail. Additionally, EATA works with Madison-Area Urban Ministries and Probation and Parole to operate the FoodShare Employment and Training Re-Entry program , provides case management services to several local non-profits operating their own individualized FSET training programs; contracts with the WorkForce Development Board of South Central Wisconsin to partner with other service providers through the WorkSmart Network; and provides training services to consumers using DVR services. Currently we operate an adult family home for four seniors that need nursing care. In 2018 we look forward to our next stage of development. Eight of these will be for seniors who wish to age in place in a dynamic, diverse neighborhood, and 4 will be affordable housing for low SES families with young children who are interested in participating in a supportive community.

Describe your organization: FoodWorks is a community-service organization founded on a strong belief in the central role of food in our lives and its power to affect a broad social mission: economic development, job creation, public education, community-building, and skill-sharing. FoodWorks will build and maintain networks in pursuit of this mission and will support other like-minded food-oriented institutions, economies, ecologies, and entrepreneurs in and around Madison. Our roots are in entrepreneurship (both in and out of food), small-business, and in the Slow Food movement.
"First Course" is our three-week job-training program for professional kitchens, designed to provide un- and under-employed adult students with the basic kitchen skills, confidence, and connections they need to get good jobs in food service. First Course covers cooking skills, knife skills, professional kitchen basics and traditions, food safety, and soft skills like job seeking strategies and interviewing practice. All graduates get a $1000 bonus to incentivize completion and help offset their opportunity cost, and we are committed to guiding all graduates to curated job leads that set them up to pursue their goals and long-term success.
FoodWorks is a start-up: we received a grant from the city of Madison at the end of 2016 to develop our curriculum and execute a pilot. In the spring of 2017 we graduated our first 4 students and 50% are working at full-time or more than full-time above Madison's living wage. In 2017 we had a successful crowdfunding initiative, and are using those funds to proceed on turning our project into a sustainable organization. Our second test of the class is currently underway with eight students, who will graduate January 25th.
FoodWorks is built upon collaborative and mutually supportive relationships with stellar community service organizations like Just Bakery, Urban League of Greater Madison, Latino Academy of Workforce Development, Open Doors for Refugees, Nehemiah, etc, as well as Madison's thriving food-based business community of chefs, producers, kitchen managers, cooks, and entrepreneurs. FoodWorks is also built upon the social networks of its founders: Jonny Hunter of Underground Food Collective, Chandra Miller Fienen of Starting Block Madison, and Matt Feifarek of Slow Food Madison.
During 2018 we plan to run 3 more rounds of classes, fully publish and support our open-sourced curriculum, further professionalize the organization, and formalize our systems for finding and admitting students, and for connecting graduates with good jobs.
Describe your ideal board member: In many ways, FoodWorks is built upon relationships with two key communities: the communities of people that we serve, and the community of food-based businesses who employ our graduates. As such, board candidates who have deep and genuine roots and/or cultural competency within communities that experience chronic un- and under-employment are ideal. We hope board candidates can build, strengthen, and support the ongoing collaborative relationships with our community partner organizations over the long haul, so that we may work together to serve them as they serve their communities. Other strong candidates will have deep roots in the "food scene", particularly from an employer/manager/food-service-worker perspective, with a side helping of passion about social justice and community service.
We'd also love to invite people who have:
- experience working within start-up businesses/not-for-profits
- connections within the galaxy of allied institutions in Madison
- understanding of funding sources, options, and strategies that support our workforce development work
- familiarity with existing employment related resources and institutions

In Madison and throughout Wisconsin health education and empowerment have helped thousands of families and health care providers. Describe your organization: The Friends of the Waisman Center, a 501(c)(3) nonprofit, supports the Waisman Center, University of Wisconsin-Madison, through fundraising, volunteer efforts, and community outreach. One of only 14 centers of its kind in the United States, the Waisman Center is dedicated to the advancement of knowledge about human development, developmental disabilities, and neurodegenerative diseases through research, service, training, and outreach. The Friends organize several community outreach and fundraising activities each year that include a benefit concert, a golf outing, and a 10K/5K/1Mile run, walk and roll for all abilities.

Describe your organization: Simpson Street Free Press, Inc. provides high-impact, core subject academic instruction. Students read extensively, write and publish, and inspire peers. High school students work as editors, advisors, mentors, and reading specialists. SSFP graduates, now in college, supervise. Teams of credentialed volunteers provide expert assistance. We flood SSFP newsrooms with practical, one-on-one academic support. SSFP students enter high school and college prepared and confident.
We publish newspapers. Our writers are kids. Known for rigorous curriculum, SSFP programs are designed to spark academic success and address achievement gaps. We emphasize writing-proficiency and literacy, and provide excellent math support. Our approach to instruction is backed by research and data. We are community-based, streamlined, and efficient. SSFP also operates a network of youth book clubs and Dane County's first all-academics youth center.
Describe your ideal board member: An ideal Simpson Street Free Press board member is interested in achievement gaps and disparities that plague Dane County communities. In Madison (in particular) after-school and summer programs aimed at low-income youth usually do not focus on academics. We do. Describe your organization: Mission: To provide leadership for continuous resource improvement through strategic partnerships that benefit the watershed's land, water and people.
The Upper Sugar River Watershed Association (USRWA) serves as the steward of the Sugar River and its upland environments, and encourages projects and behaviors that enhance the quality of these resources. The Upper Sugar River Watershed, with a drainage area of approximately 170 square miles and 115 stream miles, is located in Southwest Dane County. It is rich in resources, including fisheries, wildlife habitat (including rare and endangered species), native plant communities, and recreational opportunities.
USRWA currently has one full-time executive director and seven board members with the goal of having between 9-11 board members.
Organizational goals:
- Work collaboratively with the farming community to reduce agricultural runoff entering the river.
- Strengthen the capacity of the USRWA to effectively fulfill its mission.
- Gather essential data to foster improvements in the watershed.
- Improve water quality through targeted restoration projects.
- Engage and educate our members and the community to increase their awareness and appreciation of the Upper Sugar River.
Over the past 12 months, USRWA has accomplished the following:
• Completed year two of the Farmers for the Upper Sugar River; a farmer-led coalition focused on reducing ag runoff and improving water quality. The group expanded to 20 active farmers, representing thousands of watershed acres
• Completed a partnership with UW-Whitewater to create an erosion model of the watershed to highlight areas of focus for future projects
• Trained 11 new volunteer monitors to maintain 26 water quality monitoring sites around the watershed
• Tested an additional six sites for total phosphorus from May-October, totaling 21 sites over the last three years
• Began year one of a citizen-based research study to assess local storm ponds through dragonfly surveys
• Relocated 300 Plain Pocketbook mussels to the Sugar River and are beginning a long-term research study on mussels in the Sugar River
• Built and installed 30 boot wash stations and new signage at river access points around Dane County to combat the invasive New Zealand mudsnail found in Badger Mill Creek
• Partnered with WDNR to restore Sugar River Wetlands State Natural Area through nine work days in 2017
• Working with Mt. Horeb School District to create locally-based environmental educational modules/curriculum for school districts and service clubs to use
• Improving river trail signage at public access points through a partnership with REI Madison and Dane County
• Held the 3rd annual Rob's Sugar River Ramble; an annual fundraising event that expanded from 164 to 220 participants in year three and netted over $13,000 for USRWA
Describe your ideal board member : USRWA is currently looking to recruit 2-4 new board members. Above all, USRWA is looking for board members who are passionate about the environment and improving the overall health of the Upper Sugar River Watershed. Regardless of skill set or background, USRWA would benefit from board members who are willing to be outgoing, make connections in the community on behalf of the organization, and contribute about five hours per month towards the USRWA's mission. Additionally, the Board of Directors is looking to expand its experience in grassroots organizing, fundraising, financial management, and non-profit management.

Our goal is to house Homeless people in Tiny Houses in a cooperative community. We currently have 5 formerly homeless residents. Each resident lives in their own Tiny House. We are working to raise enough money to make improvements to the Village and to expand the Village to 9 residents. Fundraising has included donations, grants and proceeds from various products made by residents and volunteers in our on-site woodworking shop and sold in our Village store. The members of the Board of Directors are collectively responsible for ensuring effective and fiscally sound practices and operations. We provide vision, leadership, advice, and direction to the members, residents, and volunteers of the organization.

Describe your organization: TCM is an ecumenical Ministry of Presence in solidarity with the residents of the Triangle Neighborhood, located in downtown Madison. The Ministry, which is staffed by a chaplain, a registered parish nurse and an outreach development manager, assists and supports residents in achieving physical, emotional, and spiritual well-being.
